Immigration Physical and Medical Exam:

Now that you have made the decision to become a United States citizen, it is Dr. Friedberg's job as a Certified Civil Surgeon to ensure that your medical requirements are completed. Our office will assist you in determining which vaccines you require and provide medical services related to immigration. In addition, we will ensure that you have all of the proper forms required to complete the medical portion of your application. Dr. Friedberg will give you an immigration physical examination, shot record review and/or vaccination administration, and then provide you with a completed Form I-693 to go with your change of status application.

About Civil Surgeons:

Most applicants for adjustment of status are required to have a medical examination. The medical examination must be conducted by a Certified Civil Surgeon who has been designated by the United States Citizenship and Immigration Services. Immigration regulations state that each Civil Surgeon selected shall be a licensed physician with no less than 4 years of professional experience. Dr. Friedberg has met all of the requirements and is a Certified Civil Surgeon.

Vaccine Requirements:

United States law requires that all applicants for lawful permanent residence obtain certain vaccinations prior to a status change. Our office will review your vaccination records and administer any vaccines required based on your age.

Please bring any shot records from a prior physician with you to your appointment.

Required Vaccines Include (Depending upon your Age):

Haemophilus Influenza Type B (Hib)Hepatitis A

Hepatitis B

Influenza

Measles

Most vaccines are available in our office and are all required along with any other vaccines recommended by the

Advisory Committee for Immunization Practices (ACIP).

We understand that you may be under a deadline to get your medical exam completed!

If you need an immigration medical physical and exam, we make every effort to see you the same day.
